TO ALL ELIGIBLE EMPLOYEES OF CONTRIBUTING EMPLOYERS:
The Trustees are pleased to present this Summary Plan Description summarizing the benefit program as revised and in effect on November 1, 2006.
The Fund’s health benefits (HealthLink Open Access III, Open Access Medicare Plan and Value Plan) are described in this Summary Plan Description. The benefits provided under the Plan are funded by contributions from Contributing Employers and Participants in accordance with the terms of the underlying Collective Bargaining Agreements. Employer and Participant payments are deposited into a Trust Fund, which is administered by a Board of Trustees, on which your Union and Contributing Employers are equally represented.
The Trustees employ these contributions to provide benefits, pay the expenses of operating the Fund and maintain reserves in keeping with the resources of the Fund. The Trustees may change, reduce or eliminate benefits in the future, either raising or lowering them, as the Trustees deem prudent. They may change the contribution amounts and eligibility rules.
