May 23, 2013   TABLE OF CONTENTS   3.1 Schedule of Benefits for Employees   Weekly Benefit Amount -    66-2/3% of weekly earnings up to a maximum benefit of $425 per week (less FICA withholding) for the first 26 weeks of disability.    50% of weekly earnings up to a maximum benefit of $320 per week for the next 26 weeks of disability. Benefit of Partial Week of Disability -    One-fifth of the weekly benefit amount for each working day of disability. Benefits Commence -    Injury -Fourth working day of disability (See Waiting Period below).    Illness-Fourth working day of disability (See Waiting Period below). Waiting Period -    Three working days of disability. Maximum Duration of Benefits –    Weekly disability income benefits will be limited to 26 weeks unless you are determined to be unable to work at any substantial gainful occupation.
