2.2.3 Continuation of Life Insurance During Disability |

If you become totally disabled before age 65, your insurance will remain in force, while you are so disabled, for one year subject to proof of such disability. If you die within that year, proof of such disability must be filed with the Insurance Company within one year of your death.
Your coverage will then continue after one year without charge provided proof of the continuance of total disability is submitted to the Insurance Company within one year of your last day of active work.
Extended Death Benefit
If you become totally disabled after age 65, the Trustees will pay the premiums to keep your insurance in effect. You must present proof of disability to the Trustees within the same time periods required by the Insurance Company, stated above.
If you do not submit the required proof of disability within the time periods stated above, you will lose your life insurance coverage.
You are considered "totally disabled" if you are unable to perform the material and substantial duties of any occupation for which you are qualified by education, training or experience, and it is expected that this condition will last for at least 180 days from your last day worked as an active employee or you have been diagnosed with a life expectancy of 12 months or less.
During the first two years following the date you qualify as Disabled under this provision, the Insurance Company has the right to have you examined at reasonable intervals. Thereafter, the Insurance Company will only require an annual examination to confirm that you continue to be Disabled. If you fail to submit required proof or refuse to be examined as required by the insurance company, your coverage will terminate.
If for any reason, you are no longer Disabled, you will no longer qualify for coverage under this provision. If you return to active work, your coverage be reinstated subject to the Plan’s eligibility requirements. If you do not return to active work, your life insurance coverage will terminate unless you exercise the conversion privilege as described below. |
