  2.2.2 Beneficiary
  You may name your own beneficiary(ies) by filing a beneficiary designation card with the Fund Office.

You may file a new card with the Fund Office, which will revoke any older card. If you have not filed a card, or if no person entitled to receive the benefit under the terms of the card survives you, your life insurance benefit will be paid according to the terms of the applicable group insurance policy. The current group insurance policy provides that the company will pay:

1. up to $2,000 to any party deemed entitled because of payment of burial expenses; and/or
2. the executors or administrators of your estate; or
3. your surviving relatives in the following order:
a. all to your surviving spouse; or
b. if your spouse does not survive you, in equal shares to your surviving children, or
c. if no child survives you, in equal shares to your surviving parents.

If a minor does not have a legal guardian, payment may be made to the person deemed to be caring for and supporting the minor. Payment will be made in monthly installments of not more than $200.

The life insurance company will pay death benefits into a checking account which is owned by the beneficiary. A lump sum payment may be elected by writing a check for the full amount in the checking account.
