  1.5 Failure to Meet Prevailing Contribution Rate - Self-payment Requirement
  In the event that any contributing employer pays contributions to the Fund at a rate lower than the rate most commonly paid by the other contributing employers (the "prevailing rate"), the Trustees may, at their sole discretion, require the Employees of that employer, as a condition of their continued eligibility for the benefit programs provided by the Fund, to make monthly Self-Payments in an amount determined by the Trustees up to the difference between the monthly prevailing rate and the monthly amount paid by their employer.

For example, if the prevailing rate is $7.04 per hour for the first 80 hours per month, the Trustees may require the Employees of an employer who is contributing $6.04 per hour for 80 hours per month to make Self-Payments of up to $80 per month.

The Trustees shall decide by resolution when Self-Payments shall be required under this Section 1.5 and shall notify the affected employer and Employees of the amount of the Self-Payment required. Failure to make any required Self-Payment prior to the end of a 60-day grace period following the date such payment is due for any month shall retroactively terminate the right of an Employee and his Dependents to benefits under the Plan as of the last day for which the applicable contributions were paid in full. Self-payments made under this provision must be continuous. If coverage terminates due to non-payment of premiums within the 60-day grace period described above, coverage under this Self-Pay provision will not be reinstated.
