  15.4 Assignment of Benefits
  Medical benefits provided under the Plan will be paid directly to the Physician or Hospital rendering services or furnishing supplies for which benefits are payable under the Plan unless you furnish proof of payment satisfactory to the Plan. Medical benefits other than those for services of Hospitals and Physicians may be assigned by you to the person or institution rendering services or furnishing supplies for which benefits are payable under the Plan. The Trustees are not required to inquire into the validity of any such assignment, and any payment made in accordance with any such assignment and in good faith by the Trustees will discharge the obligation of the Trustees to the extent of such payment.

Prescription drug benefits for drugs purchased at an Express Scripts pharmacy using the Express Scripts card will be paid directly to the pharmacy. Prescription drug benefits for drugs purchased at a non Express Scripts pharmacy or at an Express Scripts pharmacy without using the Express Scripts card will be paid to the Participant.

Dental benefits will be paid directly to the provider upon receipt of the Fund’s claim form with a signed acceptance of the Fund’s assignment of benefits from the dentist. By accepting assignment of benefits, the dentist agrees not to bill patients for any amount in excess of the usual, customary and reasonable amount, as determined by the Fund.

Life and Weekly Disability Income Benefits are not assignable.
