  13.1 Health Assessment
  The following health assessment tests are available through H & H Health Associates:

  • Initial Health Assessment -- blood pressure measurement, a Health Risk Appraisal questionnaire, a Comprehensive Health Survey and appropriate counseling to provide baseline information to you so that you can see where lifestyle changes could be made to improve your long-term health status. The Initial Health Assessments will be offered at worksites (with the permission of your employer), at the GCC Local 6-505M office and at the office of H & H Health Associates, 11132 South Towne Square, Suite 107, St. Louis, MO 63123.
  • Comprehensive Health Survey (CHS) – consists of 46 blood tests including: Glucose for diabetes, total cholesterol, triglycerides, HDL and LDL cholesterol, Cholesterol/HDL ratio for heart disease, plus tests for kidney disease, electrolytes, liver, heart and bone damage, iron, hemochromatosis, thyroid disease, immune system, leukemia, anemia, etc. This panel of tests at Quest Laboratories costs $332.
  • Cardio CRP – a test that measures the amount of inflammation of the blood vessel wall. If the vessel wall is irritated, it increases the formation of blood clots. Due to the inflammation of the wall, the clots are unstable and tend to rupture, travel down the vessel wall and lodge in the artery causing a heart attack or stroke. The test detects those individuals who “fall through the cracks” because they are not suspected to have problems with heart disease if they have good cholesterol and blood pressure levels. For those individuals being treated for elevated cholesterol levels, it help the physician to know how aggressive to be with the treatment regime. This test at Quest laboratories costs $70.50.
  • Prostate Specific Antigen (PSA) – a test for prostate disease including prostate cancer. This test is for male employees 40 years of age or older. If you are only doing the PSA test, there is no need to fast. This test at Quest Laboratories costs $96.75.


Eligibility Active (Open Access III) Participants, non-Medicare Retiree (Value Plan) Participants and Open Access Medicare Participants.

Initial Health Assessment -- $10 CHS & PSA* -- $10 Cardio CRP -- $20

*Note: If you are an Open Access Medicare participant in the St. Louis Graphic Arts Joint Health and Welfare Fund, you should obtain your PSA test through your regular physician. Medicare covers this test, but because H&H Health Affairs is not a Medicare provider, Medicare will not cover it if it is performed by H&H.

If you are a Medicare Retiree participant in the St. Louis Graphic Arts Joint Health and Welfare Fund covered by the Medicare Complete HMO, you should obtain these tests through your Medicare Complete primary care physician because H&H Health Affairs is not a Medicare Complete provider and Medicare Complete will not cover the tests or the flu shot if they are performed by H&H.
