13.0 WELLNESS PROGRAM FOR EMPLOYEES, RETIREES AND DEPENDENTS |

Introduction
The Trustees have contracted with H & H Health Associates to provide wellness services to you and members of your family. H & H Health Associates is an organization separate and independent from the Fund. The Fund pays a fee to H & H Health Associates to make its wellness services available to Participants.
The costs for all of the services offered through the Wellness Program are greatly reduced compared to prevailing fees for such services rendered outside the Program. If you wish to schedule an appointment for one or more of the Wellness Program services or receive additional information regarding such services contact H & H Health Associates at (314) 845-8302. |
