1.1.12 Continuation of Eligibility Due to Disability – Medical, Dental, Prescription Drug and Wellness |

A Participant who is entitled to receive weekly disability income benefits from this Fund (even if such benefits are reduced to zero because of receipt of other benefits) will remain fully covered under the Plan with respect to himself and his eligible Dependents without being required to Self-Pay until the last day of the second calendar month following the calendar month in which eligibility for the weekly disability income benefits cease. If such a Participant applies for and is approved for Social Security Disability Benefits, the Participant’s coverage (and that of his eligible Dependents) will be further extended without the necessity of Self-Payment to the date the Participant becomes entitled to Medicare. If a disabled Participant does not qualify for Social Security Disability Benefits, he must elect COBRA Continuation Coverage beginning with the third month immediately following the month in which his weekly disability income benefits cease or his health benefits with the Fund will terminate. Subject to the terms described in Section 1.1.9, COBRA Continuation Coverage may be continued for up to 18 months (or 29 months if applicable) from the date coverage would otherwise terminate under this provision.
