  TABLE OF CONTENTS
  1.1.8 Events Resulting In Termination Or Loss Of Coverage – Life Insurance
  Employee life insurance coverage will terminate on the earliest of the following:

1. At the end of the first month after any month during which an Employee fails to acquire 70 Contribution Hours;

2. In the case of an Employee who receives retirement benefits under a retirement plan provided under a collective bargaining agreement to which Local 6-505M is a party, the later of:

   i. the effective date of commencement of retirement benefits, or

   ii. the end of the first month after any month during which he fails to acquire 70 Contribution Hours;

3. The first day the Employee becomes covered as an employee under another group plan that provides life insurance benefits comparable to those provided under this plan;

4. The date the Employee is no longer in a class eligible for benefits;

5. The date the employer terminates the Employee’s employment;

6. Termination of any applicable group insurance policy with respect to coverage under that policy, unless the Employee is covered under the policy’s extended benefits for disabled individuals;

7. The date the Trust Fund terminates.
