D. BENEFITS APPLICABLE TO ALL PARTICIPANTS |

1. EMPLOYEE ASSISTANCE PROGRAM (EAP)
The Health and Welfare Fund has contracted with H & H Health Associates Employee Assistance Program (EAP) to provide counseling for all Plan Participants and their covered Dependents who need assistance with personal problems such as emotional distress, marital or family problems, alcoholism, drug abuse, legal problems and financial difficulties. If you or a member of your family need help with respect to a problem of this type, you may contact the EAP at (314) 845 8302 or via e-mail at info@hhhealthassociates.com to arrange for a pre-paid consultation. H & H Health Associates is an organization separate and independent from this Trust Fund. The Trust Fund pays a fee to H & H Health Associates to make its EAP services available to all active and retired Participants and their Dependents.
2. WELLNESS PROGRAM
The Fund offers wellness services for Plan Participants and covered Dependents at discounted costs through H & H Health Associates. Available services include routine physical examinations, mammograms, flu shots, smoking cessation programs and health assessments in addition to classes on other health issues. These services are described in detail in Part XIII. |
