Effective January 1, 2008 – Wellness Program Changes
  Section D.2 in the Synopsis of Benefits describing the Wellness Program on page 19 of the Summary Plan Description of November 1, 2006 is amended to read as follows:

D.2 WELLNESS PROGRAM

The Fund offers wellness services for Plan Participants and covered Dependents including routine physical examinations, mammograms, flu shots, and health assessments. Some services are provided through H & H Health Associates. The wellness services are described in detail in Part XIII. Some benefits are not available for retirees or for individuals covered through an insured HMO plan.


Part XIII, starting on page 129 of the Summary Plan Description of November 1, 2006 is amended in its entirety to read as follows:

Introduction The Trustees have contracted to provide wellness services to you and members of your family. Some of the services are provided by H & H Health Associates, an organization separate and independent from the Fund. The costs for services offered through H&H Health Associates are generally reduced compared to prevailing fees for such services outside the Program. If you wish to schedule an appointment for one or more of the Wellness Program services or to receive additional information regarding such services contact H & H Health Associates at (314) 845-8302. Review the benefit summary carefully because some benefits are not available for retirees or for individuals covered through an insured HMO plan.


13.1 Initial Health Assessment
An initial health assessment is available to new participants through H & H Health Associates consisting of
  • a blood pressure screening;
  • a Health Risk Assessment through a personal wellness profile;
  • a Comprehensive Health Survey (CHS) consisting of 46 blood tests including: Glucose for diabetes, total cholesterol, triglycerides, HDL and LDL cholesterol, Cholesterol/HDL ratio for heart disease, plus tests for kidney disease, electrolytes, liver, heart and bone damage, iron, hemochromatosis, thyroid disease, immune system, leukemia, anemia, etc.; and
  • appropriate counseling to provide baseline information to you so that you can see where lifestyle changes could be made to improve your long-term health status.
The Initial Health Assessments will be offered at worksites (with the permission of your employer), at the GCC Local 6-505M office and at the office of H & H Health Associates, 11132 South Towne Square, Suite 107, St. Louis, MO 63123.

Eligibility New Active Participants within a year of initial eligibility. This benefit is not offered to Dependents.

Cost: $10 to the participant. Additional blood tests are available through H&H if you want to order them. You are responsible for the full cost of these additional tests.

13.2 Annual Exam/Wellness Program

Once a year, in your “birthday month” you are entitled to a wellness review at the H & H Health Affairs’ office at 11132 South Towne Square, Suite 107, St. Louis, MO 63123.. In the alternative, you can have a wellness review at a health fair at Local 6-505 or as otherwise offered by the Fund. The annual wellness review consists of
  • a blood pressure screening;
  • a Comprehensive Health Survey (CHS) consisting of 46 blood tests including: Glucose for diabetes, total cholesterol, triglycerides, HDL and LDL cholesterol, Cholesterol/HDL ratio for heart disease, plus tests for kidney disease, electrolytes, liver, heart and bone damage, iron, hemochromatosis, thyroid disease, immune system, leukemia, anemia, etc.; and
  • for male employees 40 years of age or older, a Prostate Specific Antigen (PSA) test which is a test for prostate disease including prostate cancer. If you are only doing the PSA test, there is no need to fast.
Eligibility
Active, non-Medicare Retiree (Value Plan), and Open Access Medicare Participants and the Dependents of these Participants. If you are an Open Access Medicare participant, you must obtain your PSA test through your regular physician. Medicare covers this test only when performed by a Medicare provider, Medicare will not cover it if it is performed by H&H.

If you are a Medicare Retiree covered through the Welfare Fund by an insured HMO, you should obtain these tests through your primary care physician.

Cost
Cost to the individual $10. Additional blood tests are available through H&H if you want to order them. You are responsible for the full cost of these additional tests.

13.4 Smoking Cessation Class

The Fund supports your access to the American Lung Association EASE (Employers Assisted Smoking Elimination) program. This is a systematic approach to quitting smoking involving a logical progression from awareness of the smoking habit to actual behavior change. Contact H & H Health Associates at (314) 845-8302 if you are interested.

Eligibility
The Smoking Cessation Classes are available to all Participants and Dependents.

Cost
The Fund will pay 50% of the cost of the program. If you attend eight of the nine sessions, you will be reimbursed the other 50%, regardless of whether you have quit smoking. In addition, the Fund will reimburse those who attend eight of the nine sessions for the cost of the nicotine patches. Reimbursement for nicotine patches will be limited to one treatment series during the Participant’s (or spouse’s) lifetime.

13.5 Mammograms

Eligibility
Participants and Dependents are eligible for partial reimbursement of the cost of routine mammograms or mammograms needed for or in connection with an existing Illness. Routine mammograms are limited according to the following age guidelines:

Age 40 and over:
one mammogram every year

Under age 40:
mammograms are covered if the treating Physician states in writing that the mammogram is medically necessary

Cost
The Health and Welfare Fund will pay a portion of the cost of a routine mammogram for individuals who meet the above criteria, as described in Section 4.3.4 (C) on page 73.

Claims for all mammograms should be submitted to the Claim Administrator in the customary manner.

13.6 Flu Shots

Flu shots are available in the fall of the year through H & H Health Associates. They are offered at the GCC Local 6-505M office and at the offices of H & H Health Associates, 11132 South Towne Square, Suite 107, St. Louis, Missouri 63123.

Eligibility
Open to all Participants and Dependents. Medicare Retirees covered by an HMO, should consider getting their flu shots through their primary care physicians.

Cost
The cost of the flu shots is not covered by the Fund, the individual pays the full cost.
