| Effective November 1, 2006 Coordination of Benefits with Medicare Parts A and B. |
| AMENDMENT |
Section 9.1 of the November 1, 2006 Summary Plan Description is amended by deleting the words “Medicare and” from item (d) of the definition of Plan.
Section 9.7 of the November 1, 2006 Summary Plan Description is amended by adding the following sentence at the end of the last paragraph on page 111.
This Plan will coordinate its benefits with benefits paid or payable by Medicare Parts A and B in accordance with the provisions of paragraph 1 of Section 9.2 above.
