Part IV of the Plan, Paragraph P of Section 4.3.4 beginning on page 78 of the November 1, 2006 Summary Plan Description from "Specialty Drugs" to "Specialty Drugs Dispensed or Administered by a Physician or Hospital."
Part V of the Plan beginning on page 88 of the November 1, 2006 Summary Plan Description is hereby amended effective May 1, 2007 by adding the following new Section 5.6 and renumbering the current Sections 5.6 through 5.8 as 5.7 through 5.9. |

Section 5.6 Specialty Drugs Prescribed by a Physician for Home Use |

Certain specialty drugs that are prescribed by a Physician for home use will only be covered when obtained from Express Scripts' specialty pharmacy, CuraScript. These drugs will not be covered if obtained from a participating retail pharmacy or from the Express Scripts mail order pharmacy.
Specialty drugs are drugs that are used in treating serious illnesses and conditions such as cancer, hemophilia, growth hormone deficiency, multiple sclerosis and rheumatoid arthritis and are sometimes administered by injection or infusion. These drugs can be very expensive. If your doctor prescribes a specialty drug for you, please contact Express Scripts at (866) 848-9870. Express Scripts can tell you whether the drug is a specialty drug subject to this limitation and, if it is, how to obtain the drug from CuraScript.
