Section 4.3.4 - Covered Charges with Special Limitations - HPV Vaccine |
| Effective December 1, 2006, vaccines for Human Papilloma Virus (HPV) will be covered when administered in accordance with generally acceptable medical guidelines by an in-network HMO or PPO provider. The deductible and co-payment will be the same as for other covered services. There is no benefit if the vaccine is administered by a non-network provider. |
