  My physician wants me to have a baseline mammogram at age 35 as per new guidelines, will this be a denied claim?
  Routine Mammograms are limited according to the following age guidelines:

Age 40 and over: One mammogram every year

Under age 40: Mammograms are covered if the treating physician states in writing that the mammogram is medically necessary
