| | I didn’t know I had a deductible or co-pay. Why didn’t you tell me? |
| | The Fund has an obligation to furnish every participant in the Plan with a Summary Plan Description. This booklet outlines in detail the benefits of your Plan. Everyone must take the responsibility to read and understand the benefits of their Plan. When plan changes occur, the Fund Office notifies all eligible participants by mail. Therefore, it is very important for you to read all correspondence sent by the Fund Office.
If you receive any infortmation that you do not understand, please call the Fund Office and ask that these changes be explained to you.
