| | Why do I have to supply the Fund Office with a copy of my marriage license, divorce decree or children(s) birth certificates? |
| | The Fund Office needs accurate records for effective dates of spouses and children due to marriage, divorce and adoption or the birth of a child.
When a divorce occurs, the Fund must determine whom the child lives with and if their is a court ordered medical responsibility to one of the parents. The Fund must also know if either of the natural parents are remarried, and if so, does the step-parent have medical insurance that covers the child as a dependent. These factors help in determining which Insurance Plan is primary, and if there is more than one Insurance Plan.
