| | If you are enrolled in the HealthLink Open Access III or the Value Plan, you will have I.D. cards issued in the participant's name only. If you are enrolled in the Open Access Medicare Plan or the Secure Horizons Medicare Complet Plan, each individual will have their own I.D. Card. |
