| | If my (or my dependent's) coverage ends, when will I receive notice about continuation (COBRA) coverage? |
| | The Fund Office is required to notify you within 14 days after receipt of notification that a Qualifying Event has occurred. If you lose coverage for failure to acquire the required number of contribution hours you will receive notice the 1st week of the 3rd month after you received your last 70 contribution in hours. |
