| | How do I add newly acquired dependents to my existing coverage? |
| | You must notify the Fund Office within 30 days of acquiring a new dependent. You will be required to complete an Enrollment Change Form to add your dependent. If you are single, actively working, and wish to add a newly acquired dependent, you must also contact your employer so that they pay the correct contribution to the Fund Office on your behalf.
