If you are an active or COBRA participant, you are enrolled in the CIGNA Open Access Plan.
If you are retired and are not eligible for Medicare, you can be enrolled in the CIGNA Open Access Plan or the Fund’s self-insured Value Plan.
If you are a retiree and are covered by Medicare, you can be enrolled in the Fund’s self-insured Medicare Wrap Plan or the United HealthCare Medicare Advantage Plan.

The Fund has an obligation to furnish every participant in the Plan with a Summary Plan Description (SPD). This booklet outlines in detail the benefits of your Plan. Everyone must take the responsibility to read and understand the benefits of their Plan. When plan changes occur, the Fund Office notifies all eligible participants by mail. Therefore, it is very important for you to read all correspondence sent by the Fund Office.

For generic drugs, you will be charged a 20% co-pay of the retail cost of the drug. Brand name drugs will be a 35% co-pay of the retail cost of the drug.
There is a list of catastrophic drugs that are limited to a maximum co-pay amount of $100.00. If you would like to know if your drug is in this category, you may contact Evergreen/IPC at (636) 614-1344.
It may be wise for you to shop and compare prices of specific drugs that you take in order to receive the lowest co-pay possible. Always remember to inform your physician when he writes a prescription that you have a percentage co-payment and that you wish to try a generic or one of the less costly drugs that he feels may benefit you.

You may also download this form from the ‘Forms’ section of our website. The form must be completed by you and your physician and returned directly to J.W. Terrill to be processed.
